Every exercise is chosen for a reason, dosed like a medicine, and progressed as your body adapts. Safety & Contraindications

This treatment is not suitable for everyone. Tell your physiotherapist about your full medical history: it is screened for at your first assessment.

  • Unexplained severe pain, night pain or recent significant trauma: these need medical assessment before loading exercise begins.
  • Acute fracture or suspected fracture in the area to be exercised, until cleared by your doctor.
  • Active infection, fever or acute inflammatory flare (for example a hot, swollen joint): exercise is deferred or modified until it settles.
  • Unstable heart disease, uncontrolled blood pressure or recent cardiac event: exercise needs medical clearance and careful dosing.
  • Red-flag symptoms with back pain, such as new numbness in the saddle area or changes in bladder or bowel control, require urgent medical referral, not exercise.
  • Recent surgery: exercise is not contraindicated, but it must follow the operating surgeon’s protocol and stage of healing.

Expected Timeline

Exercise therapy works on the timescale of tissue adaptation, not overnight relief. Many patients notice they move more freely and trust the painful area more within the first two to three weeks. Measurable strength change typically takes six to eight weeks of consistent training, because muscles and tendons adapt gradually. Your physiotherapist will explain what is realistic for your specific condition at the first visit.

Expect a formal reassessment roughly every two to three weeks. If objective measures(range, strength, balance times, pain with defined tasks) are improving, the programme is progressed. If they are not, the diagnosis and plan are revisited rather than repeating the same exercises indefinitely. Mild, short-lived muscle soreness after sessions is normal; sharp or lasting pain is a signal to adjust the dose, and you are encouraged to report it.

For long-standing problems such as arthritis or recurrent back pain, the aim is to make you independent: clinic visits taper off while your home or gym programme continues. What is the difference between exercise therapy and going to the gym?

A gym programme aims at general fitness; exercise therapy treats a diagnosed problem. Exercises are selected after a clinical examination, dosed to your tissue tolerance and stage of healing, and progressed against measured baselines. The supervision matters too: technique faults that are harmless in a healthy gym-goer can aggravate an injured joint or tendon.

Should I see a doctor or a physiotherapist for exercises?

For most muscle and joint problems you can come directly to a physiotherapist. How many sessions will I need at the clinic?

It varies with the condition, how long you have had it, and how quickly you progress. Many patients attend more frequently for the first two to three weeks while the programme is built and technique is learned, then taper to weekly or fortnightly reviews. The aim is always to make you independent with a home programme, with reassessment points agreed in advance.

Do I have to do exercises at home as well?

Yes: the home programme is where most of the adaptation happens. Clinic sessions teach, correct and progress the exercises; the repeated practice between visits builds the strength and mobility. We keep home programmes short and realistic, usually 15 to 25 minutes, and adjust them if your work schedule or symptoms change.

Can exercise therapy help me avoid surgery?

For several conditions(including knee osteoarthritis, many rotator cuff problems and most back pain) a proper trial of exercise-based treatment is recommended before considering surgery. Some patients improve enough that surgery is no longer needed; others proceed to surgery stronger and better prepared. No honest clinician can promise you will avoid an operation, but a structured trial gives you a genuine choice.

I am an athlete. Do you do sport-specific rehabilitation?
