What is Lumbar Spondylosis?

Lumbar spondylosis is a general term for age-related degeneration of the spine in the lower back (lumbar region). This condition involves wear and tear of the spinal discs, joints, and vertebrae that naturally occurs as we age. While it's a normal part of aging, the degree of degeneration and associated symptoms can vary significantly between individuals.

As we age, the water and protein content of the spine's cartilage changes, resulting in weaker, thinner, and less flexible spinal discs. This degeneration can lead to other spinal problems such as disc herniation, bone spurs, and narrowing of the spinal canal (spinal stenosis), all of which can cause pain and neurological symptoms.

An important and reassuring point: the changes seen on an X-ray or MRI correlate surprisingly poorly with pain. Imaging studies of people with no back pain at all routinely show disc degeneration, bulges, and other spondylosis features, especially with increasing age. This is why we treat the person in front of us(your movement, strength, and daily demands) rather than the scan report. It is also why a "degeneration" finding on a report is not a reason to fear activity or to stop working, riding, or exercising.

Among our patients in Jaipur, two lifestyle patterns come up again and again: long unbroken hours at a desk, and daily two-wheeler commutes over uneven roads, both of which load a stiff, deconditioned lower back in exactly the ways it dislikes. The answer is rarely to give these up; it is to break up sustained postures, build the strength and mobility the spine needs to tolerate them, and adjust the small details(chair height, riding breaks, lifting habits) that add up over a working week.

The Progression of Lumbar Spondylosis

  • Disc Degeneration: The first stage typically involves the loss of water content in the discs, reducing their shock-absorbing capacity and height
  • Disc Bulging: As discs flatten and weaken, they may bulge outward, potentially pressing on nearby nerve roots
  • Facet Joint Arthritis: The joints that connect vertebrae (facet joints) experience increased pressure and develop osteoarthritis
  • Bone Spur Formation: The body responds to joint instability by forming bone spurs (osteophytes) to stabilize the spine
  • Ligament Changes: Spinal ligaments may thicken and become less flexible, further limiting movement

Risk Factors

  • Age (primary factor – most people over 60 have some degree of spondylosis)
  • Genetic predisposition to spinal degeneration
  • Previous spine injuries or surgeries
  • Occupations requiring heavy lifting or repetitive spinal movements
  • Obesity, which increases pressure on the spine
  • Smoking, which can reduce blood flow to spinal discs
  • Sedentary lifestyle with poor posture
  • Sports or activities with high spinal impact

When to Seek Professional Help

You should consult a physiotherapist for lumbar spondylosis if you experience:

  • Persistent lower back pain lasting more than 2 weeks
  • Pain radiating down one or both legs
  • Numbness, tingling, or weakness in your legs
  • Difficulty controlling bladder or bowels (seek immediate medical attention)

Did You Know?

  • Spondylosis changes are found on scans in a large majority of older adults, yet many have no symptoms at all. The presence of degeneration on an X-ray or MRI doesn't necessarily mean you'll experience pain.

  • Staying active with appropriate exercises is one of the most effective ways to manage lumbar spondylosis, contrary to the old belief that rest is best.
