Asthma & Cystic Fibrosis
Respiratory physiotherapy supports people with chronic lung conditions, and its role is specific and bounded. At Modern Physio we focus on understanding the root-cause of your problem & personalizing your treatment for lasting results.
Understanding Asthma & Cystic Fibrosis
Respiratory physiotherapy supports people with chronic lung conditions, and its role is specific and bounded. It does not treat asthma or cystic fibrosis — those are medical conditions managed with medication under a doctor's care, and inhalers and prescribed treatment must continue. What physiotherapy contributes is breathing pattern retraining, airway clearance techniques, and exercise capacity: helping clear secretions, making breathing more efficient, and building the fitness that makes daily life easier. For cystic fibrosis in particular, airway clearance is a recognised part of daily management.
Common Symptoms
- Breathlessness on exertion, or at rest in more severe disease
- Persistent cough
- Difficulty clearing sputum from the chest
- Wheezing
- Chest tightness
- Reduced exercise tolerance
- Fatigue from the increased effort of breathing
- Recurrent chest infections
How Common Is It?
Asthma is one of the most common chronic conditions worldwide. Cystic fibrosis is far less common but requires intensive daily management. Both benefit from physiotherapy alongside medical care rather than instead of it.
Causes & Risk Factors
- Asthma: airway inflammation and narrowing in response to triggers
- Cystic fibrosis: an inherited condition causing thick secretions in the airways
- Bronchiectasis and other chronic lung conditions with retained secretions
- An inefficient breathing pattern developing alongside chronic disease
- Deconditioning from reduced activity, which compounds breathlessness
- Environmental triggers, including air quality and dust
Our Diagnosis Process
- Reviewing the medical diagnosis and current treatment from the treating doctor
- Assessment of breathing pattern at rest and on exertion
- Assessment of chest wall movement and posture
- Assessment of cough effectiveness and ability to clear secretions
- Assessment of exercise tolerance within safe limits
- Immediate referral where symptoms suggest deterioration or infection
Our Treatment Approach
- Airway clearance techniques taught for daily home use — central to cystic fibrosis management
- Breathing pattern retraining, including diaphragmatic breathing
- Positioning to assist drainage and ease breathlessness
- Graded exercise to build capacity within safe limits
- Posture and chest-wall mobility work
- Coordination with the treating doctor, particularly around timing relative to nebulised medication
Recovery & Prevention Tips
- Keep taking prescribed medication — physiotherapy adds to it and never replaces it
- Do airway clearance consistently, at the times agreed with your medical team
- Stay active within your limits; deconditioning makes breathlessness worse in a self-reinforcing loop
- Learn positions that ease breathlessness so you can use them before panic sets in
- Seek urgent care for worsening breathlessness, fever, chest pain, or a change in sputum colour or volume

Can physiotherapy replace my inhaler?
No, and this matters. Asthma and cystic fibrosis are medical conditions requiring prescribed treatment. Physiotherapy supports breathing efficiency, secretion clearance and fitness. Stopping or reducing medication is a decision for your doctor alone.
What is airway clearance?
A set of techniques — breathing patterns, positioning, and sometimes devices — that help move secretions out of the airways so they can be cleared by coughing. In cystic fibrosis it is a recognised part of daily management, taught for home use.
Is exercise safe with asthma?
For most people with well-controlled asthma, yes, and it is beneficial. Exercise is introduced gradually, with your reliever inhaler available and your doctor's guidance on triggers. Poorly controlled asthma should be reviewed medically before increasing activity.
When should I seek urgent care rather than physiotherapy?
Worsening breathlessness, breathlessness at rest, chest pain, fever, coughing blood, or a marked change in sputum colour or volume all need urgent medical assessment. Physiotherapy here works alongside your treating doctor, not instead of them. Modern Physio does not prescribe medication and has no imaging on site; where medical review or investigation is the right next step, you will be told so and referred.
Can you treat children with cystic fibrosis?
Airway clearance is taught to families for daily home use, and technique changes as a child grows. This is always done in coordination with the specialist team managing their care.
