Crutches are prescribed when a specific weight-bearing restriction has to be respected, most often after a fracture, ligament repair or lower limb surgery. Your surgeon or physiotherapist will specify non-weight-bearing, touch weight-bearing, partial weight-bearing or weight-bearing as tolerated, and the crutch technique differs for each. Axillary crutches, the underarm type, are the most familiar and give good stability for short-term use. The critical safety point is that your weight must go through your HANDS, never through your armpits. Resting your body weight on the underarm pads compresses nerves in the armpit and can produce a genuine crutch palsy with wrist and hand weakness. There should be a two to three finger gap between the pad and your armpit.

Elbow crutches, with a forearm cuff, are lighter, easier to manoeuvre and far better for longer-term use. They demand more arm strength and coordination initially, but most patients who need crutches for more than a few weeks are better served by them.

Stairs are where most crutch accidents happen. The rule that keeps you safe is simple: going up, the good leg leads; coming down, the crutches and the injured leg lead. We practise stairs in the clinic before you attempt them at home.

How to Choose & Use Crutches

  • Take your weight through your HANDS, never your armpits. Keep a two to three finger gap under the arm.
  • Going up stairs: good leg first, then injured leg and crutches. Coming down: crutches and injured leg first, then good leg.
  • Follow the exact weight-bearing instruction you were given. Non-weight-bearing means the foot does not touch down at all.
  • Keep crutches slightly out to the side rather than directly in front, and take modest steps rather than long ones.
  • Check tips for wear regularly and avoid wet or loose surfaces until your technique is confident.
